CD with any CT/MRI/X-ray images and the associated reports. Failure to bring the CD may result in your appointment being rescheduled.
- Please complete your patient paperwork prior to your scheduled appointment. If you do not receive your paperwork by mail or email, you may print and complete the packet for your scheduled provider by clicking on your provider’s name to the left. Failure to complete paperwork before your appointment may result in your appointment being rescheduled. If you are unable to print the forms, please arrive 20–30 minutes early to allow time to complete them.
- You may be asked to change into a gown and/or shorts for your examination. For your comfort, you may choose instead to follow these recommendations:
- Neck: sports bras for females
- Back: non-denim shorts, sports bras for females
- Shoulder: Sports bras for females
- Elbow/Wrist/Hand: ability to access the shoulder
- Hip: non-denim shorts
- Knee: non-denim shorts
- Ankle/Foot: ability to see the knee
- Nerve Testing (EMG)

Plan for your visit
If you are unable to keep your scheduled appointment, we kindly ask that you give us 24 hours’ advance notice. If you are more than 10 minutes late for your appointment, you may be rescheduled for a different date and time. Patients who no-show or late-cancel (anyone who fails to give at least 24 hours’ notice) are subject to missed appointment fees. These fees are not billable to insurance and must be paid prior to scheduling future appointments.
If you need to be seen for more than one body part, please let us know as soon as possible so we can best accommodate the time needed for your appointment.
Study Participants Needed for Low Back Pain Research
We are seeking participants for a research study titled “Ultrasound Assessment of the Thoracolumbar Fascia in Individuals with and without Low Back Pain, and Treatment Response with OMT or Hydrodissection.” This study aims to improve the understanding of the thoracolumbar fascia’s role in low back pain. Participants will undergo a physical exam and ultrasound imaging, with some receiving Osteopathic Manipulative Therapy or hydrodissection treatments. The study is conducted by VCOM in collaboration with Virginia Tech, funded by the DoD and AOA, with compensation provided.
